For Freshmen

  • Entrance examination (NO ENTRANCE EXAM FOR SY 2023-2024)
  • One (1) ID picture (1×1, latest)
  • Form 138
  • Certificate of Good Moral Character
  • Birth Certificate from NSO (photocopy)

For Transferees

  • Entrance examination (NO ENTRANCE EXAM FOR SY 2023-2024)
  • One (1) ID picture (1×1, latest)
  • Honorable Dismissal
  • Scholastic Records / Copy of Grades
  • Certificate of Good Moral Character
  • Clearance for Admission from the Registrar’s Office

Nursing

The baccalaureate degree program is a four year nursing program, where the clinical duty starts on the second year. It follows the CMO 14, S. 2009 Curriculum from the Commission on Higher Education.

Entry requirements for Freshmen

  • Form 138
  • Birth Certificate from NSO
  • 1 x 1 ID picture (2 pcs.)
  • Certificate of good moral character

Screening Requirements for 2nd Year

  • At least a minimum grade point average of 2.15
  • Passed the Nursing Aptitude Test /Battery Exam (NAT)
  • Passed the Psychological Test
  • Personal interview
  • Possess the Carative Elements of a Health Caregiver
