
Applicants to the Bachelor of Science in Nursing are required to take the
Nursing Aptitude Test (NAT).
REQUIREMENTS
- Two (2) recent 2×2 ID photos with white background and name tag
- Photocopy of latest Report Card (Form 138) – for Grade 12 students or graduates; Required GWA: 90% or above
- Certificate of Good Moral Character
- Photocopy of Birth Certificate issued by the Philippine Statistics Authority (PSA) or National Statistics Office (NSO)
- Payment of Entrance Exam Fee
- Certificate of Rating (For ALS/PEPT Passers)
- Copy of Grades / Transcript of Records (For Transferees)
- Apostilled Transcript / Scholastic Records (For Foreign Students)

Reminders:
- Please pay the required down payment to be officially enrolled. Paying less than the required amount will not process the enrollment.
- Students who are still waiting for their complete original copies due to on-going classes will be asked to execute a promissory letter stating when they can submit the required documents.

STEP 1 Register at mytuaportal.com
– Click New Applicant
– Fill out the Admission and Registration form; Click Submit
– Check your email for confirmation and next steps
STEP 2 OAR Issues Clearance to Pay
OAR will provide the official Clearance to Pay for the NAT.
STEP 3 Pay the NAT Fee
Settle your exam fee at the Finance and Accounting Office (FAO).
STEP 4 Take the NAT on the Scheduled Date
Complete the Nursing Aptitude Test (NAT) as scheduled.
STEP 5 Receive Exam Results
STEP 6 Complete Enrollment Process
– OAR will provide the Certificate of Admission
– Open mytuaportal.com
– Proceed to Registration Module
– Review your Student Assessment Form (SAF)
– Click “Save” to generate registration assessment
– Click the “+” (plus) icon
STEP 7 Pay Tuition Fee at the Cashier
