Dr. Tristan Jourdan C. Dela Cruz, RN, is a respected nursing educator and practitioner with extensive experience in academia and healthcare. Currently serving as Program Chair of the Graduate School of Nursing and Ethics Chair at TUA’s Institutional Ethics Review Committee (IERC), Dr. Dela Cruz is deeply committed to shaping future nursing professionals and advancing the field. Holding the rank of Associate Professor IV at the St. Luke’s College of Nursing (SLCN), he brings a wealth of expertise in knowledge advancement, teaching, learning, and service. He is an active member of Sigma Theta Tau, an esteemed international community of nurses dedicated to fostering communities of practice, education, and research in nursing. With a background as a Former Staff Nurse at St. Luke’s Medical Center (SLMC) – Quezon City and a Former Dispute Resolution Officer at IBM Philippines, he offers invaluable insights bridging theory and practice.

  2. Honorable Dismissal (from last school attended)
  3. Transcript of Records / Scholastic Records
  4. Certificate of Employment (proof of at least 1 year clinical experience)
  5. Photocopy of PSA / NSO Birth Certificate
  6. Photocopy of Marriage Certificate (for married female applicants)

II. Procedures

ONSITE

STEP 1 Initial Evaluation at the Graduate School of Nursing

  • Visit the Graduate School of Nursing Office for evaluation
  • Present your updated PRC card and Certificate of Employment (proof of at least 1 year of clinical experience)
  • Upon passing the evaluation, the secretary will provide you with the list of admission requirements

STEP 2 Fill out Registration Form at the Office of Admission and Registration (OAR)

To facilitate the application process, the student may encode in advance his/her information via https://mytuaportal.com

STEP 3 Bring your Transcript of Records / Scholastic Records to the Graduate School of Nursing for evaluation.

STEP 4 Submit the following documents to the OAR:

  1. Updated PRC Card
  2. Honorable Dismissal (from last school attended)
  3. Transcript of Records / Scholastic Records
  4. Certificate of Employment (proof of at least 1 year clinical experience)
  5. Photocopy of PSA / NSO Birth Certificate
  6. Photocopy of Marriage Certificate (for married female applicants)

OAR Records Examiner inspects, validates, and encodes credited subjects to the TUA portal.

STEP 5 Coordinate with the Graduate School of Nursing (slcn_gs@tua.edu.ph) for assessment.

Review the generated Student Assessment Form.

STEP 6 Pay Tuition Fee at the Finance Office to be officially enrolled.

  1. Please pay the required down payment to be officially enrolled. Paying less than the required amount will not process the enrollment.

STEP 1 Initial Evaluation at the Graduate School of Nursing

  • Email your updated PRC ID and Certificate of Employment (proof of at least 1 year of clinical experience) to the Graduate School of Nursing at slcn_gs@tua.edu.ph
  • Wait for 2-3 days for the result of the evaluation.
  • If qualified, you will receive an email containing the Office of Admission and Registration (OAR) admission requirements.

STEP 2 Open https://mytuaportal.com

  • Click New Students
  • Fill out the Admission and Registration form; Click Submit
  • Check email for the login link

Log in as follows:

  • Account type: Applicants
  • Email Address: (enter email address provided)
  • Password: tuapassword (this is the default, kindly change once logged-in)
  • Term: FIRST SEM OF SY 2024-2025
  • Click Generate Student Number

STEP 2 Email the Graduate School of Nursing and attach a copy of your PRC Card, Transcript of Records / Scholastic Records for evaluation.

STEP 3 The Graduate School of Nursing will forward the evaluation to the OAR Records Examiner. 

  • The OAR Records Examiner inspects, validates, and encodes credited subjects to the TUA portal.
  • Visit the Dean’s Office for the copy of your evaluation.
  • Proceed to the Trinitian Guidance and Career Center (TGCC) for an interview.
  • Submit the following documents to the OAR:
  1. Updated PRC Card
  2. Honorable Dismissal (from last school attended)
  3. Transcript of Records / Scholastic Records
  4. Certificate of Employment (proof of at least 1 year clinical experience)
  5. Photocopy of PSA / NSO Birth Certificate
  6. Photocopy of Marriage Certificate (for married female applicants)

STEP 4 Coordinate with the Graduate School of Nursing for assessment.

Review the generated Student Assessment Form.

STEP 5 Pay Tuition Fee

  • Proceed to Payments
  • Choose your Payment Mode; Input Amount to Pay;Click Pay Now
  • Select your Payment Option; Click Send Instructions via Email/Mobile
  • Click Validate Payment

After payment, please wait for at least two (2) to three (3) days for the Finance and Accounting Office’s (FAO) clearing and checking. After which, the student may check his/her portal account to check if he/she is Officially Enrolled.

Reminders:

  1. Please pay the required down payment to be officially enrolled. Paying less than the required amount will not process the enrollment.
  2. Scanned copies of admission credentials should be attached as part of the ONLINE. However, original copies must be submitted to the OAR Admission Officer on or before the first day of classes. Failure to comply may nullify the admission.

Once officially enrolled, email the Graduate School of Nursing at slcn_gs@tua.edu.ph for additional information such as:
